In Iowa, Parental Consent must be provided on a written consent form either in-person at a driver’s license DOT office or complete the Parent’s/Guardian’s Written Consent Form (form 430018) and bring it to the DOT office

You must get a lot of driving practice with a parent, legal guardian, an immediate family member who is older than 21 or a person at least 25 years with written permission from your parent, guardian or custodian. The supervising adult must possess a valid driver's license. You must also complete a Driver’s Education course, hold a valid instruction permit for a total of at least 12 months and maintain a clean driving record for at least six consecutive months immediately before applying for an intermediate license
